I/WE declare that we have read and understood a Concussion Awareness Resource for the appropriate age group (Age 10 & Under, Age 11-14, or Age 15 & Older) as required by Ontario Law for anyone under the age of 26 to be able to participate in organized sports.

I/We declare that we have read, understood and agree to abide by the East York Curling Club's Removal From Sport, Return to Sport Protocol.
